Getting Your Childhood Cancer Tag is Easy!
• Print the form below and fill it out
• Take the form and your current tag to your County Treasurer's office
• Pay required fees and $50 donation
You can see where we have supporters with our distinctive license plate. You can help us fill the entire state GOLD by getting a childhood cancer license plate. Every plate helps our Fueling the Fight initiative to get gas cards to Kansas families.
Please reach us at heidi@lovechloe.org or (785) 515-2211
if you cannot find an answer to your question.
Visit your county treasurer's office to purchase a Love, Chloe license plate. The Treasurer's office will collect an annual tax-deductible gift of $50 that will go to the Love, Chloe Foundation to provide gas cards to Kansas families navigating a childhood cancer diagnosis. The $50 gift is in addition to normal tag fees and taxes. All of these will be paid together in one transaction and payable to your Treasurer's office.
You may purchase the Love, Chloe license plate at any time of the year. If you choose to purchase your plate at a time other than your annual tag renewal period, you will be required to make a second tax-deductible contribution of $50 when your normal renewal timeline comes around.
Your tax-deductible gift to the Love, Chloe Foundation will allow us to provide gas cards to Kansas families through our Fueling the Fight program. The cards will help cover gas expenses while traveling to and from childhood cancer treatment.
The State of Kansas does not allow personalization of distinctive license plates or requests for specific plate numbers. The Love, Chloe childhood cancer plate will be produced consecutively by number and issued randomly.
No. Unfortunately, even if you renew your tag online, you'll need to go to your local Treasurer's office to request the Love, Chloe childhood cancer license plate. Once you've done that, it will be mailed to you and should arrive within 30 days.
If you have any questions or encounter problems (which we don't foresee happening), you can ask your Treasurer's office staff to contact Sarah Bayless, the Vehicles Services Supervisor for the Department of Revenue.
Her contact information is:
Phone: 785.296.2228
Email: sarah.bayless@ks.gov
You will need to take your current plate in the Treasurer's office to exchange it for your Childhood Cancer tag. You will receive a 30-day tag to put on your vehicle until your distinctive license plate arrives or approximately 2-weeks.
